Forms Checklist
It is important to submit a number of important forms to make your visit to SHINE 2012 go smoothly. This is particularly important for our international attendees.
All Students Receiving Financial Support
All students receiving any financial support (registration fee waiver, airfare, lodging) are required to complete and submit the University of Hawaii WH-1 Form (click to download).
Foreign Students from a US Institution
Foreign students from a US institution on F visas are required to complete and submit the following forms: WH-1, W-8BEN (if applicable), copy of passport, copy of visa, copy of I-94, copy of I-20 from host institution, copy of permanent resident card (if applicable).
Foreign Students from a Foreign Institution
Foreign students from a foreign institution are required to complete and submit the following forms: WH-1, W-8BEN (if applicable), copy of passport, copy of visa, and copy of I-94 (issued upon entry to US).
Foreign Post-docs and Scientists
Foreign post-docs and scientists receiving any financial support are required to complete and submit the following forms: WH-1, W-8BEN (if applicable), copy of passport, copy of visa, copy of I-94 (issued upon entry to US), and FMIS-45.
Once your financial assistance is approved, send original signed forms and copies of documents to:
